Automatic Payment Authorization.pdf (68k) - Establish automatic payment from your financial institution.

Cancellation Form.pdf (155k) - Cancel the account and request refund on funds in the account.

Change of Address.pdf (56k) - Change address for contributor and/or beneficiary.

Change of Beneficiary.pdf (65k) - Change beneficiary on account.

Change of Contributor.pdf (58k) - Change contributor on account.

Intent to Enroll (163k) - Required only if the beneficiary is attending a private or out-of-state school.

Joint Contributor.pdf (46k) - Add a joint contributor to account.

Release on UTMA/UGMA Funds Rollover.pdf (87k) - Required if you are transferring funds from a UTMA or UGMA to account open a SCTPP account.

Rollover Form (52k) - Required if you are moving funds from either another 529 Qualified Tuition Program, a Coverdell Education Savings Account, or a qualified US Savings Bond to redeposit the funds within 60 days into a SCTPP account.

Scholarship Refund Request Form(pdf) (136k) - Request refund due to scholarship.  No partial scholarship requests are accepted.

Social Security Number Add/Change Form.pdf (154k) - Add or change a social security number on account if it is missing or incorrect.
